If the term "low grade" or "high grade" is indicated as the grade for a urothelial primary, assume it is a WHO/ISUP grade.
Note 2: If morphology is not urothelial, code 987.
Note 3: If another grade system other than WHO/ISUP is documented for a urothelial primary, code 999.
